Prior to your appointment
Health Intake Forms
Please print and complete the Health Intake Form.
In-Person Consultations: you may bring your completed and signed form with you to your appointment.
Virtual Consultations (online): please submit your completed form to wellness@keralaayurveda.us at least 24 hours in advance of your appointment.
Please note that if you choose to submit the form via email, you are agreeing to sharing your personal and medical information electronically.
Please note our cancellation policy
Appointments may be cancelled via email, or by calling +1 (510) 257-6640 ext. 121 at least 24 business hours in advance. A $50 charge will be incurred for those not cancelled within that time frame.

Please arrive at least 10 minutes before your appointment start time.

Online Consultations
How to setup your account for virtual appointments
- You should receive an invitation from Zoom with your appointment confirmation. If you cannot find it, please contact us at wellness@keralaayurveda.us to request one.
On the day of your appointment, log into your Zoom 5 minutes early to prepare for your consultation.
